On your first visit, please bring the following:

• Referral from your primary care physician, if required

Appointments

We see patients by scheduled appointment only. Scheduled appointments allow us to better serve your needs to keep waiting times at a minimum. For your convenience, our office hours are Monday, Tuesday and Friday from 1:00 p.m. to 5:00 p.m. and Thursday from 9:00 a.m. to 6:00 p.m. Our telephone number is 330.386.9212.

Please call as far in advance as possible for an appointment. If you are unable to keep it, please call 330.386.9212 to cancel so your appointment may be given to another patient.

Insurance Information

Sunil Parulkar, M.D., Urology is a participating physician in most all major insurance plans. However, it is important to check your insurance providers list of participating physicians to be sure your program allows you to be seen at our office. Also, please review with your insurance provider the rules and limitations on coverage and reimbursement

It is important to always carry your insurance information with you. Our staff will process insurance forms as a courtesy and convenience for you. In order to expedite the processing of your claim, it is your responsibility to provide us with the proper form or insurance information. Before submitting an insurance form, be sure you have completed your portion and have signed it.

Referrals

If you are insured by a managed care plan, it is your responsibility to obtain your referrals from your primary care physician. This pertains to all visits, including follow- ups.

Prescriptions and Refills

Please have your pharmacy phone number available at the time you call the office with a medical problem or refill.

Between visits, you may wish to call regarding special problems. Whenever possible, please call during office hours.

Fees and Billing

Charges for visits vary depending upon the services rendered. Payment is required at the time services are rendered. Co-payments are expected at the time of your visit.
